I believe the idea that Montreal was the 10th cleanest city came from an article in Forbes which used as its source a report prepared by Mercer Human Resource Consulting.

A (summarised) definition of &quot;clean&quot; in this report is as follows:

&lt;b&gt;Scores are based on the quality and availability of hospital and medical supplies and levels of air pollution and infectious diseases. The efficiency of waste removal and sewage systems, water potability and the presence of harmful animals and insects are also taken into account. &lt;/b&gt;

Montreal&#039;s climate means that it has to face and solve many problems that otherwise lead to unsanitary conditions and poor health. This is does well and should be commended - I guess it comes down to your definition of clean !</description>
